Department of Family Medicine Receives Fan Fox Grant



The Department of Family Medicine is the proud recipient of a $300,000 grant from the The Fan Fox & Leslie R.Samuels Foundation

Improving Health Outcomes in Homebound High Risk Vulnerable Elders in the South Bronx identifies older persons who would benefit most from a Home Based Primary Care Program (HBPC).  Our HBPC Program coordinates and provides enhanced care for homebound elderly in the Central and South Bronx by utilizing Community Health Workers to provide social and peer support for patients, educate patients in self-management practices, empower patients to take an active role in their personal health, help link patients to appropriate services in the healthcare organization and community, and provide feedback to providers regarding patient status and how to decrease barriers to care.   Under the direction of a Board-Certified Geriatrician, the HBPC team will also include a Geriatric Nurse Practitioner, a Social Worker and Social Work Interns, and Resident Physicians. 

This exciting program is slated to start on June 1,2009

At one point or another, every Department at Bronx-Lebanon Hospital Center comes in contact with the public.  With that contact comes the opportunity to present a positive image to our community.  The Department of Family Medicine is most often responsible for providing patients with their impressions of this Department.

Our Department works very hard on guest relations.  Patients and families coming to our Department are often in a stressful situation.  So, our job is to ease concerns in a caring and efficient manner.

                 New Initiative:

The Department offers various Workshops such as prenatal care, Diabetic classes, and Smoking Cessation.  A research forum meeting is also offered once a month providing a focused discussion to assist in moving projects forward by providing constructive criticism, astute observations, and profound remarks.  Having this meeting helps residents when they submit their study to get IRB approval.  Also, several doctors in the Department have participated in training to become Focus Group Facilitators to improve our research efforts.

Also, on the second week of July, 2009 Dr. Hartman started a Family Planning Clinics in the afternoon at two siteFulton and Ogden clinics, this will offer birth control options counseling, IUD's, the NuvaRing, Depo, and Implanon to patients.

The FEGs WeCare clinic has joined our team and had helped our department increase the Volume of patients seen each day.  With the increased patient volume, the Department was awarded a HRSA grant approved for $803,000.00 over 2 years.  This grant will help the Community Health Workers bring more patients into care.

We are also pleased to announce the launch of a Geriatric program under the guidance of Dr. Scaccia.  Sponsored by the Fan Fox and Leslie R. Samuels Foundation, the Home Based Primary Care Program is available to Homebound High Risk Vulnerable Elders in the community.

The Family Wellness Center, a collaborative initiative of the Dr. Marting Luther King, Jr. Health Center and Bronx-Lebanon's Departments of Family Medicine and Psychiatry, opened in February, 2008.

The Wellness Center's program emphasis is on helping patients requiring repetitive hospitalizations for chronic illness.  Since April, 2009, Ms. June Howard, P.A. has began working in the Wellness Center.  In addition to conducting FEGS Reviews, she conducts the Detox Admission Histories and Physical exams.

Family Medicine, under the guidance and steady hand of Dr. Slezinger at Fulton with support from Dr. Martinez at Ogden as well as all attendings and residents are commited to expanding the quality and quantity of HIV care that we provide in all our health centers.

Great things have been happening at Ogden.  Dr. Martinez started a Morning Incentive program for the community.  With Janice Void, Residency Administrator, and the help of Coach for Living Donation, they had given several Incentives to patients that arrive early to their appointments.  They have raffled a toaster oven, electric razor, bathroom organizer, etc.  Their patients are very appreciative and the turn over has increase.
